COVID-19/Coronavirus Waiver

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.


NWA Conservatory of Classical Ballet (“The Conservatory”) has put preventative measures in place to reduce the spread of COVID-19; however, The Conservatory cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending The Conservatory could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Conservatory and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at The Conservatory may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Conservatory owners, employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my or my child(ren)’s attendance at The Conservatory or participation in Conservatory programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless The Conservatory, its owners, employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Conservatory, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Conservatory program. 



COVID-19 In-Person Camp/Intensive Procedures
  • The main double doors are the “IN” doors. Please enter the building through the main double doors.
  • The single door through the gate (Studio B hallway) is the “OUT” door. Please exit the building through the single door.
  • A parent may walk their child into the building (we ask that all adults please wear a mask while in the building) for check-in. At this time, our waiting areas, observation room, Shire and Man Cave are closed. Parents may leave and return, run errands or wait in their car during lesson time. A parent may re-enter the building at dismissal time to receive their child.
  • Students will have their temperature taken with a no-touch infrared thermometer upon checkin. Children with a temperature reading above 100 degrees F will be asked to return home.
  • Students should bring all of their dance necessities, including a water bottle, in ONE personal bag. Each student will have an assigned bin for their personal items inside the classroom. Students will remove their street shoes prior to walking onto the studio floor. They will take their street shoes and bag to their bin. At that point, they can remove their cover up and put on appropriate dance shoes.
  • Each student will have an assigned barre spot and center spot. Students will be directed as to how across the floor exercises will be completed in order to maintain a safe distance from other students.
  • We encourage students to wear a mask until they have reached their classroom spot in the studio. At that time, they are free to leave their mask with their personal items. If the parent/student desires that the mask be work during the class, that is perfectly fine.
  • Hand sanitizing stations will be available outside of each studio. Students should sanitize hands on their way in and on their way out.
  • Bathrooms will be disinfected often. It is advised that students be sure to use the restroom prior to coming to the studio, if possible.
  • Barres, personal item bins, sound equipment, bathrooms, door knobs, and other high touch areas will be disinfected often.
  • Instructors will wear masks when in close contact with students. When they are able to keep more than 6 feet away, they may opt to pull their mask down so that instructions may be heard more easily.
  • If you can answer “Yes” to any of the following, you should NOT enter the building:
    • A temperature of 100.4°F or above for adults and 100°F or above for children
    • Signs or symptoms of COVID-19 (fever, cough, shortness of breath, difficulty breathing, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell)
    • In the past 24 hours, has the parent or student entering the building taken medication to reduce a fever
    • In the previous 14 days have you had contact with someone who has a confirmed diagnosis of COVID-19
    • In the previous 14 days have you travelled outside of the United States or to areas within the United States with widespread transmission
